help compose a letter, please

i have a bit of a dilema...

my rh emailed me that they need the proof of insurance from my caterer, and they need it NOW.

i had fh call the caterer. she was in school and said she would call back. SHE HAS NOT!!! we're going on day three. i think something happened to her cell phone because she has ALWAYS been good about getting back to us.

i don't have an email address for her, but i do have a fax#.

i could use a little help in composing a note to her about the situation. i don't want to sound angry (she will be handling my wedding food... don't want her spitting in it) and i don't want to sound wimpy.

please help.

She's a professional, and if she has insurance, this is a standard request, and should not be something new to her.

Simply write:

Dear....

Our hall has asked for proof of insurance for all our vendors. Please call the hall at .... to submit your information, as it is an urgent request.

Thank you for your time and understanding in advance.

I am sending this fax to you as I have not heard back from you after leaving messages on your cell phone. Mountain Lakes House, the reception hall for my October 7th wedding which you are catering, is requesting proof of insurance from you. I need to turn this in soon so that they have it otherwise, they will not allow you to cater the affair. Please get back to me as soon as possible so that this step in the planning can be finished.
