Help, How do I make the cry packet??

I learned about the cry packets from some of the topics you lovely ladies have posted. I have even seen some beautiful pictures of cry packets you girls have made. But I can't seem to find directions on how to make them. They look easy enough, but every time I fold the paper, something doesn't look right. Does anybody have direction for making the cry packets? I just brought fancy white paper at AC Moores. I figured if I put a ribbon in the color of the girls dresses around it with maybe a white rose or something that would look nice.
Did anybody else do cry packets, did anybody use them?

Here's ours:

We followed Martha Stewart's Tissue Pouches instructions as Gail (HappyBride2005) posted link above. Instead of using cardstock we just used 24lb. paper instead. We found it's easier to work with.

We did as in MS instructions put them on all chairs. That way everyone would know about them instead of leaving them in a basket. Yes, they were used and we still are hearing about what a great detail they were...from men! Yes...Men! Men thought the tissue pouches were such a great touch!
