HELP!! Need to know how to address an invite

Help FH and I are putting addresses into computer for our invites. Do we address the 1st envelope Mr. & Mrs. Smith or Mr. & Mrs. John Smith?

The outer envelope where the address will be needs to be the most formal.

Outer is Mr. and Mrs. John Smith, the inner is John and Jane Smith

outer envelope should be Mr. and Mrs. John Smith, we did inner envelopes with what we call everyone (either by first name or by Mr. and Mrs. Smith)

I have some stupid questions.
1. Do you write out and for Mr. and Mrs. or do you put a &?
2. What if you are inviting a friend and guest (Ms. Jane Smith and Guest), what do you put on the outer and inner envelopes?

you write out the and in Mr. and Mrs. John Smith
And if you are inviting JOhn Smith with a guest then on the outer envelope you could put Mr. JOhn Smith
and on the innter envelope you can put John and Guest
If you know that John Smith has been dating Jennifer Brown seriously for some time, they you can put Mr. John Smith and Miss. Jennifer Brown on the outer, or just his name and then on the inner envelope put John Smith and Jennifer Brown.
I think if you look on Martha Stewart's website she has stuff on proper etiquette for addressing the envelopes

Mr. and Mrs. John Smith and Family for outter, and on inner:

Mr. and Mrs. Smith
Jack, Sarah, and John

The kids names are each listed on the inner envelope to avoid confusion on who's invited.
