Hotel Block Wording/Template

Anyone have a template that they used for inserting the Hotel Block information with their STD's? Even phrasing would be helpful.

Our hotel gave us cards to insert with the STD's...and then I'll just insert them again with the invitation i guess.

Otherwise I would say something like the following:

For your convenience, a group of rooms has been set aside at _________________________. In order to access the group rate of $__ please make your reservations no later than four weeks prior to or wedding date.

Dont forget to include the phone number, and also instructions on how to access the rate (i.e. a code or your last names).
