Well i can try to help on the living together couple
i feel out of respect for the couple no matter if married or not. It should be both of there names in the invite. I have gotten invitations and they only refer to my fiancee i feel disrespected by that and we end up not going. But you are inviting so is up to you and your fiancee.

for the first, you should do...
mr. john smith and mrs. jane jones

and for the second,
ms. jane jones and mr. guest (find out the name!)
timmy jones

Re: how do i address....

If you know who the guest will be I would put their name. If you don't want to then it's fine to put and guest. It might be easier that way.

In general (out of respect) you should always put the Guests name if you know it.
