How do you address the inside envelope?

I am in the process of making my guest list to send to the printer. What's the correct way to address the inside envelope? Do I include last names or just the first name??

For Example

Addressed to:
Mr & Mrs. John Smith

What would I write on the inside envelop?

This is what I did.

Family: (outter)Mr. & Mrs. Michael Smith
(inner) Uncle Michael & Aunt Joyce

Friends: Mr & Mrs. Joseph Jones
Joe (or Joseph) & Lynn

I had a similar post the other day. I've decided to do this:

Outer:
Mr. and Mrs. John Smith


Inner
John and Mary

I checked that site and it said that for unmarried couples, only the person who lives at the address is used on the outer envelope. Did anyone else follow that?

For example -
Miss Jane Doe - outer envelope

Miss Jane Doe and guest or
Miss Jane Doe and Mr. John Smith - inner envelope
hmm - I would have just put and guest on the outer envelope.

I'm putting both names on the mailing envelope for unmarried couples. I also decided to keep the inside envelope formal, using whole names and Mr. & Mrs. John Smith for married couples. It just makes it easier with Aunts and Uncles and stuff. Thanks for all your help.
