someone constantly sniffling their nose to the point where you think whatever is inside it has, by this point, made it into the back of their brain. Ugh it's so gross, especially when the person is sitting next to you on the subway.

the sound of the frame of my car vibrating when it's stopped.... it drives me insaaaaaaaaaaane!!!
When FH burps.... it doesn't gross me out, but it annoys the ever living sh!t out of me!

the garbled sound of the train conductors on the subway - we can't hear you anyway! the sirens and garbage trucks in the morning someone blowing their nose and the person's voice in the cube across from my office

nails on a chalkboard fork/knife/spoon scratching a glass/ceramic plate cardboard scraping against cardboard (I used to work at a sneaker store and some of the shoe boxes were a weird cardboard, so whenever they rubbed against each other it made this horrible annoying sound)
