I had that too and I was told since they are family member you can go ahead and do Mr. & Mrs.

That's what we did and it's my dad's cousins who are both doctors, and the placecard we did Mr & Mrs also and apparently they asked my mom about it! And on their response card they crossed our the M on the M ________ and wrote Drs. XXXXX.....hahaha....
