you could probably ask them, but i think it's usually their first names.

I hadn't thought of this but I think we will just do first names since FH's parents are divorced but she kept the same last name. Plus his mom is not remarried so they're not really Mr. and Mrs.

I think their first names followed by the last name. But my friend did it different and just did her parent's first names!
