invitation wording?

ok, it's a bit early to send them out just yet, but i want to play around with designs and to do that i need wording...

my parents, fh's mom, dad, and step dad (who his mom just divorced), and us are all contributing to the wedding. how would i word this?

also, how would i address an envelope to an engaged/long term couple who are not living with each other. (i have a few people like this. i'm good friends with one member of the couple and know the other and are friends with them because they're together. i'm assuming it'd go to #1's address).

The formal way to write it would be write your friend's name & guest so it would be Ms. Jane Smith & Guest.
