I may be wrong, but I thought if both members of the couple were doctors you address it to;

Doctors Smith.

If both are doctors then it should be

Doctors John and Jane Smith

Now to stir things up....
What is the wife is a DR?
Is it, Mr & Dr. John Smith?

Why does a woman get a last name only if she's a doctor?

Well, if she outranks him in professional title or has a different last name than him, her name would be separate and go first.

It's tradition for the man to go first if they have the same last name (professional title excepted, as noted above) and yes, it's unfortunately sexist.

On one of my reply cards, I had a woman who responded with her and her husband's names separately even though they had the same surname (Ms. HerFirstName TheirLastName and Mr. HisFirstName TheirLastName). Although not 'correct,' I think it's a graver offense not to call someone what s/he wants to be called. Their escort card reflected what they replied in the RSVP. If someone is offended by the Mrs. HisFirst HisLast tradition, buck tradition and make your preference known. A good bride and groom will pay attention.
