A couple we're inviting to our wedding--we're not sure if the bride changed her last name or not. We're not being very formal w/names, but would it be wrong in this case to use 'Mr. & Mrs XX XXXX'?

That's what we did. Unless we knew specifically otherwise we just addressed everything as Mr. & Mrs. XXXX

I am keeping my name & i don't really get bother by people sending us stuff Mr & Mrs. ..if anything they will write out how they should be addressed on their RSVP card, right?

Do you think it's okay to do XXX & XXX Johnson then? I keeping my name and adding FH's and that wouldn't bother me.

Why don't you ask them?
I didn't change my name and yes, it would bother me if the assumption were made and I received an invitation to Mr. and Mrs. HisFirstName HisLastName.
Obviously, it doesn't bother some women. But it does offend others. Why not go the safe route and make sure what her name is?
If you had contacted them a number of times already with no response, then you'd have a reason to move forward as planned.
