Where are you getting yours, and how early did you order? Is my rabbi supposed to fill it in??

it depends on the rabbi, i think ... i was told to find the text on the internet somewhere but show it to the rabbi for approval before actually getting it printed.
i haven't ordered my ketubah yet so i can't answer your other question

i had mine custom made by someone on etsy, it took her about a month to do (hand painted, came out gorgeous)
i had her paint in our names and date, etc, the only thing we will do on the day of is sign our names, witness name and rabbi name

we got ours from Zion Lion and had it personalized with our names and wedding date.
The cantor had recommended Ketubah.com. Zion Lion took FOREVER (literally... ordered August 9, we got it Dec 17)

I got mine at Lion Zion in New Hyde Park. The prices were the same as online, and they hand caligraphy portions that need to be filled in as well. HTH
