how much can they alter the dress to fit if you lose say 20 lbs or more

It depends on the dress!! If it is a gown with a lot of detail that can't be moved around than I would think not much.

It also depends on where you lose the weight. If you are wearing a ballgown and lose the weight in your thighs/hips it might not make much of a difference

It really depends on the dress. Be careful because if you go for your first fitting and THEN lose 20 lbs then they'll charge you an arm and a leg because that's a significant decrease.
