Ok, I know what kind of ID I need to get a marriage license, but what information will I need to know to fill out the forms?

Father's name and mother's name (maiden name) Parents' places of birth Your and FH's places of birth Address, phone, etc I believe we also gave social security number Whether or not you were married before
It's all stuff you should know - no difficult questions.
