what do you need to bring with you?

Birth Certificate and Passport and/or Driver's License.... You also need a money order. If you get it done in any of the 5 borrows it is $35... On the Island or Westchester it is $40

birth certificate is not required right? just a valid ID?

You need 2 forms of ID one of which being a photo ID.
