marriage license and name change

Hi,
Where do we go in Nassau County for our marriage license and what do we need to bring with us?

Is there one application I need to fill out to officially change my name , and then I go to DMV, SS, etc. ?

Please help.

u can go to any town hall to apply for the license.
if u go to hempstead, u need your drivers lisence and also passports or other form of valid photo id (other town halls require a birth cert)
it costs $40

then in the mail u will get your lisence and then at your wedding u give that to your officient and they fill it out, have your witnesses sign and they mail it in and then u get your official cert in the mail about 10 days later.

once u have your official cert, u can go to SS and DMV to officially change your name.
