What did you have to show the clerk and where did you go? I'm from Yonkers so when I called there, they need driver's license, passport and a birth certificate and I'm not sure I know where my birth certificate is!

all i needed was my drivers license and my health insurance card.

All we needed was our drivers license, but we brought our passports and birth certs just in case.

We went to the Hempstead location. They just asked me for two forms of ID. I brought my license, passport & birth certificate.

You can bring your license, passport and birth certificate.. if you dont have your birth certificate you can bring your baptisimal certificate
