I am leaving my current job soon and i liked my manager and want to give her a thank you note basically just thanking her for the opportunity... do you think its appropriate to give her a small gift, like $10 giftcard to starbucks as well?

I think that is a nice idea. Or maybe you can give her one of those cookie platters that the bakeries make for the holidays. My mom usually does that for her bosses. It's cheap, and its pretty and they get to eat them !!! hehe
I think the gift card is a good idea as well. :)

bottle of wine or cookies are really good ideas. I would probably go with the bottle of wine though. Something small seems like a really nice thing to do.
