yes we went to the post office, filled out forms, pain about $80 and it came about 5 weeks later.

you also need your birth certificate.. be prepared to have them take it... I was very suprised that they took it from me!
when i got mine in fla..the woman at the post office told me my birth certificate wasn't real!! Turns out she just didn't know what a NY birth certificate looked like! haha

When you go, you should go with your passport pictures already in hand...your local photo shop should be able to take passport phots... call around
The birth certificate came mailed back with the passport... I was super nervous about giving that up... but if it ever gets lost, you can always get another copy sent to you from the state (since I was told my birth certificate was 'fake' from the fla lady, I had another one sent to me... i think it costs $15 or something... so now I have two)
