Photo/Video Contracts-what should be included?

What should i make sure is included in our contracts for photo & video??

Who the photographers and videographers will be- Their names, not just the studio should be ON the contract.
Exactly what do you get- is it an hourly rate, what is the overtime rate. What time will they arrive and where, do they go to the ceremony and reception? Are there in-between photos/video taken at a 3rd location such as a park. Is all travel included in your cost? What time do each leave your reception-after cake cutting or the last dance?
How many proofs should you expect- a min or average is fine. Do they charge for proofs over a certain amount? WIll they take B&W and Color photos, what is the ratio?
For Photographer:
Which album will you get? There are different quality albums, so have them write the one you want. How many photos are included in that album. What are the extra charges for adding pages, or special touch-up work? Parent albums, enlargements, thank you cards, thank you photos. Do you keep the proofs? The negatives? Have that all details.
Video-
Will you get DVD or Video? How many of each? How much are extra copies? Does this price include special effects (if you want) photo montages, your choice of music or is that up to them?
