Place Card Wording

Do you do two separate place cards If somebody (not married) is bringing a date? I have no idea what the correct thing to do with place cards are??

if u know the name of their guest
Mr. John Smith
Ms. Jane Doe

If not,
Mr. John Smith and Guest
