Of all of them I would go with:

Mr. and Mrs. Michael Doe

Now just to add more confusion - I actually plan on writing out both names so:

Mr. Michael Doe
Mrs. Catherine Doe

Table #

Mind you this idea changes daily.

It depends on how formal it is:

Formal: Mr and Mrs _____

A little informal you could do their first full names.

I wouldn't do shorter or nick names though JMO

I think the correct way is - the way you wrote it out on your envelopes: Mr. & Mrs. Michael Doe

That's how I'm going to do it.

I think it depends how formal your wedding is.
I believe for a very formal wedding this would be correct:
Mr & Mrs Michael Doe

But this would also be nice and noone would complain:
Catherine & Michael Doe

It is a formal affair, but I don't want anyone getting insulted by not putting their name on the placecard.

I think I am going to go with:

Catherine & Michael Doe

But does the woman go before the man?

Yes the woman goes before the man..at least I think so

jar823- i'm doing exactly what you are doing except i am putting the man's name first on all of them. consistent with my inner envelopes on my invitations.

i am also using the name by which everyone goes by. ie; if it's my aunt cathy and i never call her catherine i would put michael and cathy doe.
