I have been trying to do it myself, but it is not coming out right.

The easiest way that I found to do it was on power point: Open three different text boxes and put in what you want.
i.e.
1- Your Name
2- His Name
3- Initial
Then all you do is play with the font, sizes and colors. You can move the text boxes and place them where you want them.
If you have an idea of what you want, save it as a picture and then just try to copy the design on Power point.
