Re: proper way to address the invites???
I've been putting 'Mr. & Mrs. John Smith' on my envelopes. And since I'm not bothering to write on my inner envelope I'll be writing '& Guest' or putting the children's names right below the parents' names on the outer envelope too. For those 'older' relatives, I don't remember if I put Ms. or Mrs. Anne Smith. I didn't write anything additional after their name. HTH
On the outers I just put Mr. & Mrs. Smitih, if it was a couple who lived together I put both their names, and on the inners I used first names.

Re: proper way to address the invites???
If you are doing inner and outer envelopes the proper way to do them (though you can in the end do it any you want) is:
Outer: Mr. and Mrs. John Smith Inner: Mr. and Mrs. Smith
Outer: Mr. John Smith Inner: Mr. Smith and Guest (preferably find out the guest's name and use that instead-but only on the inner)
Outer: Mr. and Mrs. John Smith Inner: Mr. and Mrs. Smith John Jr., Sally and Michael
Outer: Mr. John Smith and Ms. Jane Doe Inner: Mr. Smith and Ms. Doe
