In the hospital after her fall at the airport, Cannie admits only to herself that the real source of all her anger was the fact that she 'had failed Joy'. What does she mean?

I think that she means that because she was a single mom, was heavy and then fell that she failed her. She thought that she could not give Joy everything that she needed.

I took it that her father had failed her, by leaving. and she didn't want Joy to have the same issues that she did.

I think she meant that her body was supposed to be a safe place for Joy to grow until she was ready to be born and that she failed in not providing that safe place until Joy was ready. Rather, she made a scene in an airport which was unnecessary and made some rude remarks to 'the Pusher.' Plus, it is not clear that she was actually pushed, so much as she slipped in water. So even though she placed the blame on the pusher, I am not sure that she really thought the fall was the pusher's fault.
Also, the doctors told her that the fall might not have caused the problem to begin with. In which case it was a freak thing within herself. As if her body had betrayed Joy.

good point becky...i thought that she felt ...the one good thing her body did was protected the baby...which it didn't

I agree with everyone, but I also think part of it was that by going to California and not dealing with her issues (including being pregnant) she had pushed them aside and then something bad came of it. She had put herself before her pregnancy and Joy and then felt guilty about it. She also put the blame on someone else.

I think she felt that by not acting responsibly, repeatedly, she started a chain of events that ended up causing the premature birth. I think she was also dealing with issues of not being able to care for Joy, while she was in the hospital.
