i dont get what they're talking about the surname....so if I choose to put it on the certificate but never really formally change it anywhere else, is that a problem?

Nope..shouldnt be a problem. You can put Jane Doe-Smith on the certificate and still sign ur credit card Jane Doe..you can leave your SSN Jane Doe..etc..
