I put both our last names
For example, if your last name is Smith and FH's last name is Anderson put it this way:

Smith-Anderson
123 Cherry Lane
New York, NY 12345

Re: RSVP return envelope

I live with FH and our invitation vendor told us to just put my name on it. She explained it as: Even though we live together, some people are still traditional and don't go for that, so rather than bring up the issue at an otherwise happy and exciting time, just put the bride's name on the envelope. I don't know how much I agree, but whatever, the RSVPs are all coming to the right house, so whatever.

I put my name (usually it's the brides name) on the RSVP. DH lived with me at the time too.

I did it this way too, except instead of a dash in between our names, I used the & symbol, as in

Smith & Anderson
123 Cherry Lane
New York, NY 12345

That's the way I've done our address labels since we moved in together over a year ago, so it made sense for us.
