RSVP Wording - Confused!

RSVP Wording - Confused!

So I just got my proofs for the Invitations and RSVP's and I'm really confused as to how my guests will fill them out. The RSVP card states:

Kindly respond
by the fifteenth of july

M_________________
will ____ attend

First: how do they put their names in? What if there's children coming? Do they squash it all in there? How am I supposed to know how MANY of them are attending? What's if its only 3 out of the 4 when I write it to 'Smith Family'

Second: if they're attending, is it 'will check attend'?

And does this invitation wording look okay?

WITH GREAT PLEASURE WE INVITE YOU
TO SHARE IN OUR JOY AS WE,
Bride
AND
Groom
BEGIN OUR NEW LIFE TOGETHER
FRIDAY, THE FIFTEENTH OF AUGUST
TWO THOUSAND AND EIGHT
AT SEVEN O’CLOCK IN THE EVENING
CREST HOLLOW COUNTRY CLUB
WOODBURY, NEW YORK

RECEPTION TO FOLLOW

TIA!!

what i did was
The favour of your response before June 1st is greatly appreciated. We have reserved ___ seats in your honor.
M______
__ number attending ___ decling with regret

i have to manually fill in the number of seats but i figure this is going to limit down the peope who think they can invite whoever they want.

this might happen--some people are just rude and don't understand that they aren't allowed to add guests...so you might have to question a few of them and say 'noticed you put 3 instead of 2' it happens

we did

____ places have been reserved in your honour

___ accepts with pleasure

___ declines with regret

we pre-filled the # of people invited---and still had some add to that. and others who weren't going to bring a guest--changed it to a 1.

Our rsvp card had the same wording. And don't forget to number your rsvp cards (we did ours in the back corner in teeny tiny pencil) in case you have guests who forget to fill out their names and just check whether they are attending or not.

My rsvp had the same wording, but without the guests attending.
Everyone actually did fill it out correctly, but then, most of our guests were older and have done this before.
