Re: Save the Date & Invites...
We are sending STD's about 9 months in advace. I'm only giving them to very close friends/ family that I really want to be there, as well as those traveling from Out of Town (I'm including hotel information for them). I wouldn't send them to every one on the 'A-list' at that point, just in case the list changes for whatever reason. Just remember when sending them, once you send it, they HAVE to get an invite as well.
As for the invites I'm sending my 'A-list' out about 4 to 4.5months before the wedding date, with an RSVP of almost 4 weeks before the wedding. if I get a large amount of 'no' responses I will send invites to my 'b-list' as the 'no's are returned, with and RSVP date 2.5 weeks before the wedding. (I hope that makes sense and I hope my logic works).

Re: Save the Date & Invites...
You should only send out your STD to the people you are definately inviting to the wedding. So A list only. And it is not 100% necessary to send to the entire A list, but that is up to you and will depend on how many STD your budget will allow for. I think protocol says you can send them anywhere between 8-12 months prior to the wedding. If you can start looking for ideas now, order them and get them out within the next 2 months that is probably ideal.
As for invites, I plan on sending them out 10 weeks prior to the wedding. If you have an A list and a B list, make sure people on the B list are not friends with those on the A list or they will find out that some people received their invites waaay before others. Try to keep track of the 'no's from the A list, and send invites to B list as needed.
