Should my program cover include our last names?

So I spent my lunch hour working with one of our graphic designers (I work in publishing) and she designed some great favor tags and cry back bands with two pretty monograms. She also did our names for the program cover, but I don't know if I should have our full names or just our full names.

I was thinking that the cover would say

The Nuptial Mass of

Krzysztof

Should it be

Juliet Leigh RXOXOXOXOXO

&

Krzysztof Michal Fxoxoxoxoxski

or just first names?
