STD - How do I address them?

I am printing out my STD envelopes. But, I don't know how to address them. Do I put....

1) Couples First & Last Names

2) Mr. & Mrs. Last Names

3) Mr. & Ms. Last Names

4) Mr. First Name & Mrs. First Name & Last Names

5) Mr. First Name & Ms. First Name & Last Names

HELP! And what if they're not married? UGH!

If they were married i used this option Mr. & Mrs. Last Names. If they weren't married i wrote Mr. first and last name & Ms. first and last name

If married I did:
Mr. & Mrs. John Doe

If not, I did:
Ms. Jane Smith
Mr. John Doe

If a family, I did:
The Smith Family

I did *almost* the same. I'm not a fan of the Mr. & Mrs. John Doe thing (I know it's correct etiquette-wise, but my inner feminist feels that no one's first name is Mrs - JMO).

If married, I did:

John & Jane Doe

I have been putting 'Mr. Blah blah & Ms. Yada Yada' if they aren't married but serious.

If they are married, 'Mr. & Mrs. Blah Blah'

If the whole family, 'The Blah Family'
