STD Wording Confusion!

Hi ladies! I'm working on my STD and ironically enough, I'm confused on how to write the date! Which way is the correct way?

Or are all 3 wrong? lol

honestly... I've seen it ALL 3 ways.


I think when I do my STDs... it will read 12.13.14
