votive placecard template?

does anyone have votive placard template for any program?
I'm trying to make the template myself but I cant get the angles right.... I wish the votive holder was completely straight but its not..

can anyone help me please??????????

bumping in desperation

DH did it in word he resized the label to match what was on the directions in the vellum label packet. The website they gave in the directions wasnt working that great. One he got the size down they came out great.
