Wedding Program Template

I am looking to create my own wedding programs but specifically want a
5x5 folded card. (Like the picture below) Does anyone have a template for that or know how I could make them? Also, do you recommend a place to buy metallic card stock? Thanks!

definitely buy metallic card stock at thepapermillstore.com ... the aspire petallics beargrass paper is really really pretty (used it for my invitations), and they had the best price

it is $37 for 350 sheets, which was way cheaper than everywhere else I looked. I can't help with the wedding programs template though =/
