on my programs I want to acknowledge FH's mom but he does not have any relationship with his mom's new husband. I can't put stepfather, do I just put her name and mother of the groom?

Re: what do I put
I would just put mother of the groom and her name. If he is walking her down the aisle you could say Xxxx - mother of the groom, escorted by Yyyyyy.

Not everybody has programs and no, they aren't required. The purpose of them is basically to list the order of events in the ceremony (1st reading, 2nd reading, candle lighting, exchange of vows, etc) and to acknowledge people like your BP, your parents, your readers, your musicians, lost loved ones, etc.
You can choose to have or not have a program whether you get married in church or at your reception site - it's entirely up to you! Also alot of people make them themselves and some people order them. You can usually get them to match your invite style if you order with them.
