Both of you have to go and bring your driver's licenses, SS cards, and your birth certificate... and $40. ![]()

My notes from when I talked to them just say SS#, so I'm not sure if you need the card or the number. Sorry.

I called today, just need social ##. Thanks! 2 forms of ID, one being picture form of ID.

Depends where you go. We just gave them our passports and that was it. Also make sure you have the $ in a money order - at least the one in Queens would not take anything else.
