What wording did you use for donation favors?

I plan on doing a donation in my grandma's memory. I was just wondering what wording I could use. I am not good with this type of stuff.

I would probably say something like 'In Loving memory of Grandma ___, we have decided to make a donation to the _____ society, etc. etc
