You have to contact the hospital you were born in and they can mail it to you, or you can pick it up

You have to contact the township or village that you were born in. It will cost you $10 and they give it to you right then.

You have to go to the town hall in which your were bron in and they will charge you $10. I had to do it about a year ago.

Town hall/whoever handles 'vital records' where you were born

you can go to the state website and look up the vital records.. this is what I had to do because I was born in rockland county, so I couldn't go to the town hall where I was born.

I used the online option and it came pretty quickly (it was pricey though).

I was born in the Bronx in Einstein Hosp.
