i'm trying to help my mom plan her wedding (yay) ...
as her favor, she wants to do a bottle of wine and a few chocolates but we're just not sure how to wrap these together nicely ... anyone have any ideas? every way i think of just looks stupid lol

I love that idea. I'm doing the same, but I'm having them take the wine bottles on the way out at the end of the wedding. I'm also doing chocolate on the table, probably just in a small organza bag.

Probably outrageously expensive and who would actually eat the chocolate off the wine bottle? (Well, probably me. ) But it's still really cool!
ETA: Otherwise, I can only think of having the chocolates in an organza bag and tied around the neck of the bottle but I don't like that idea.

Contact Afresh Approach! They have some nice ways of incorporating and attaching their truffles to wine bottles. I saw some samples in their store, it looks nice!

I got a gift like this from a place where I use to work. The attached truffles to the wine in a tulle wrap and then it was tied around the neck of the bottle. I don't know where they got it from but prob a place like a fresh approach.
