Did you need a referral?
Was it considered an emergency that needed to be reported to the insurance company?
Did you pay your co-payment?

Some insurance companies only cover a certain percentage.

Call your insurance company and ask them why it wasn't covered ... it may just need to be resubmitted.

Did they pay any part of it? Do you have to submit to get a portion paid? Do you have a deductible that hasn't been met yet?

It could be a mistake... or it could have went towards your deductible. I work in a doctors office ... and sometimes I have problems understanding the EOB's, (Explanation of benefits)and that is after 13 years!!!! Call your insurance carrier.

A;ways call...sometimes you can get that pid if its out of network but not your choice. It happens a lot with hospital visits.

Also there are a million plans...your employer picks the plan and sometimes the plans like yours with $100 deductible and 10% co-inusrance is because you then pay less for the insurance policy itself( more out of pocket for you, so thus less cost insurance plan)
