Another name change question

When at DMV I want to change my name on the registration and title as well as my license. I also have to change my address. I found 3 different forms online for these changes. Do i fill out all the forms or can it just be the license application one? I am very confused!!!

If you go to the DMV, there is one form that does both your registration and your title...then I filled out another form for my license which changed my name and my address.

So, should only be 2 forms.
