Changing name at Social Security office
I believe it's in Freeport, can someone please confirm where it is and what I need-just the marriage certificate, right? Thank you! Gee, I've been married for 7 months now...guess I'm a little slow!!

yes-
the original marriage certificate..and if you have your SS card that would probably help too. They will give you a receipt indicating your request for the change and the new card will come in the mail.

Oh you also need you new licence.... Have the form filled out before you get there....

A Tip...
I went to the office in Lindenhurst...its a small office and they were open at 8:50 and I was the first person in and I was out at 9:05! it was pretty painless. Also, DO NOT go at the begining of the month...too many people there getting their checks.
