Re: Name change on passport
You have to include your marriage license. The website tells you all the information need. Fairly simple stuff. You can just mail it in because it's a renewal.

Re: Name change on passport
I looked it up- If you have renewed your passport within the last year all you have to do is download a form from the US Passport site - send it in with your old passport and aoriginal marriage license. there will be no fee. If you passport was renewed more than 1 year ago- you have to apply for a new one with your marriage lic and pay the full fee.HTH

Re: Name change on passport
i just did this. you can download the form off the us passport website. there is a list of fees to renew. send in this form, with a check for the renewal fee, along with your marriage license.. i got my passport and my marriage certificate back about 3 weeks later. make sure that you are done changing everything else first though before sending your marriage certificate away.ETA: you also need to include your old passport as well as new passport photos

Thanks ladies. Sounds annoying but I am happy to change my passport photo!! I was 200 lbs when I took it... LOLETA: I don't like the idea of mailing my original marriage certificate, but oh well. Going to Jamaica for a destination wedding on 11/11/11!!!!!

I didn't either....I sent it with a confirmation of receipt thing (its lime green) at the post office it was like 60 cents or something just so I didn't have to hear 'it got lost in the mail' on the way there.

Same here... plus we have a few trips planned that we booked in my maiden name so I don't want to run into any issues.

Re: Name change on passport
for me it was annoying, only because i foolishly booked an international flight in my married name before actually changing it legally. II procrastinated changing my name, and tried to change the flight back in my maiden name, but the only way you can do that is canceling the flight and re-booking it. So in order to get my passport name changed on time, I had to have it expedited (I went to the Dept of SS, DMV, and post office all in one day). But if I didn't have to rush it, the process is no big deal.
