Personalized chocolate wrapper

Personalized chocolate wrapper

Can anyone help me with personalized chocolate wrappers to be used as place cards? It would be great if I could have everyone's name and table # printed on the wrapper, but if that's not possible then just lines saying Guest:___________and Table #:__________________ that I can fill in myself (with a graphic in the background). Can this be done?
