Anyone have a DJ question list?

I need help from my fellow brides! Am going to talk to DJs this weekend...anyone have a list of question? requests? I should ask? I'd welcome any advice. Thanks!

You want to be sure they play music
continuously
Policy regarding special requests
Do you want give aways?
Additional musicians for cocktail hour?
Do you want special lighting, smoke machine, projection slide show?
Dancers?
Can you see tapes of MC/DJ's available or will they be at any bridal show or expo so you can see them in person?
Payment plan
Overtime rate
They need to be able to provide a certificate of ins to the caterer
