Please tell me how you changed your name on your certification?! I am a lil lost with how to do this

We were just talking about this at work the other day. I haven't done it yet, but I believe some co-workers were saying you have to send a name change form and a copy of your marriage license to the state. One co-worker said it was insane trying to do it with the DOE! Not sure where you work.

I wanted to make sure I heard the info right and to see how involved it was (I seem to find ANYTHING dealing with Albany is a big hoopla!). This is what the site says:
