It is a tissue in either a plastic bag, or something along those lines with a little note or poem and the bride and groom's name and wedding date.

Its not hard at all. You can print on vellum envelopes and decorate them. Put tissue in the envelope. You can print something like this on envelope:
'A wedding gown
A tux
A tie
A tissue for you in case you cry'
Or
'For your tears of joy'
'A wedding gown
A tux
A tie
A tissue for you in case you cry'
