DH and I had edible favors that were a semi-DIY. I'm not sure that's what you're looking for, but here goes:
1) Pastel French mint candies from Metro Candy.
2) Blank mint tins from Favor Online.net.
3) Circular labels from the Office Depot Web site. I designed a graphic for it.
We got all products from the Web and were satisfied with all the above vendors.

We had truffles and a dark chocolate heart - got them from my sister's store (she owns a gourmet chocolate/cheese store).

carmel, candy , and fudge covered apples from Bon Bon Chocolatier in Huntington. They were amazing and looked pretty on the table. Pics in my
