So Im giving out small favors but also as an additional favor Im donating to the Muscular Dystrophy Association....
My mother has muscular dystrophy and I want to say something like'to honor my mother' (shes alive so I dont want to make it seem like shes not)...but i dont know exactly how to word the note...

i would say something like ' to honor my mother and the many others dealing with muscular dystrophy i would like to be a part to help with research and to find a cure, and i would love it if you could help too!

i was just going to say.. 'living with'
or to keep it shorter.. 'a donation has been made in her name'

I did a search and found this wording, they put it on a pretty scroll and rolled it up and tied it with a bow and left it on each place setting, very elegant looking.
